We know the health care system can be complicated. And when you have complex health needs, we are here to provide you even more support. Speaking with someone who really knows about health care issues can put your mind at ease. Our nurses are here to serve as your personal health advocate.

CAROLINE: Before I had my knee replacement. I’ve had both knees done at separate times. But before I had any surgery, I had a nurse representing Aetna call me and ask me if I had any questions about my procedure, if there was anything that I was unfamiliar with or concerned about.

01:05 GERRY: They taught me what carbohydrates were. How to read the labels on the cans and that type of thing. It was a real learning experience for me. I’m really happy I could talk to the Aetna nurse.

01:39 PHILLIP: They said “You know, we have a place that you’d be able to get your Dexilant. They told me where to get the form. They told me the people to contact. I think they even contacted them. They didn’t have to go that far to help me with that.

A nurse can even provide in-person care management services in your home, with your consent.

If you’re interested, we can help with things such as:

  • Planning for, or coming home from, a hospital stay
  • Managing a medical condition
  • Coordinating complex medical treatment among health care providers

We hope you also take advantage of our support programs, such as:

Case management services

Case management programs are for people who need extra assistance and support. Our services help you adopt and stick with proven treatments.

If you qualify, we’ll assign you a case manager. This person will work with you and your physicians to support your care plan (available for certain medical conditions). Your nurse case manager may:

  • Help you avoid another hospital stay — If you’ve recently been discharged from the hospital, we’ll check in with you to see if you need help understanding your discharge instructions, confirm you’ve filled your prescriptions and remind you to schedule follow-up care.
  • Offer end-of-life support — We’ll provide an extra layer of support during end-of-life care that honors your dignity, needs and choices.
  • Help when you have multiple medical conditions — We’ll coordinate treatment among your doctors and provide additional resources to help you manage your conditions effectively.
Disease management

We offer a disease management program for members with certain health conditions (such as diabetes). This program supports members and helps them understand their condition and follow their doctor’s treatment plan. Members who qualify are offered:

  • A total health perspective — We see people as people, not conditions. This holistic approach helps us deliver a helpful combination of information and support.
  • Personal attention — Eligible members have access to registered nurses to answer questions about conditions and treatment.
Everyday well-being

You don’t have to have an urgent need for support. You might benefit from guidance that helps you stay well. So you may also get a call from us to:

  • Discuss questions to ask your doctor
  • Find out about Aetna® health and wellness programs that might be right for you
  • Learn about available resources and services
  • Talk about ways you can work toward good health
  • Direct you to online resources for self-care
Transplant services and support

Our National Medical Excellence Program® supports members who require a solid organ or stem cell transplant. We’ll provide you with a dedicated case manager who can help you with all of your health care needs, from the time you’re approved for a transplant through post-transplant care.

Blood pressure and cholesterol monitoring

Our Numbers To Know® program promotes the benefits of blood pressure and cholesterol monitoring. We’ll send you a reminder to check your blood pressure. You’ll also get a card to help you track blood pressure, cholesterol, medication and dosage information.
